How to use legal tools and forms in the Legal Resource Center
- Click on the “Legal tools and forms” link in the list
- Click on “Personal documents”
- Select your state and click “Go”
- Select a category (example: wills, power of attorney, estate planning)
- Click on the banner that says, “Click here to select this document”
- If you’ve used this service before you can enter your e-mail address and password. Or if this is your first visit, simply:
- Enter your email address
- Enter your choice for a password twice
- Review and agree to the terms of use
- Click “Register”
- Now you can follow the prompts to fill in the blank fields on the form before saving and/or printing a copy. Or you can download a blank copy of the form.

- Member Website: Through this website, you can search for resources online from our network of providers from dependent and elder care to pet care.
